Led by Catherine Battaglia, PhD, RN, a team of investigators with HSR&D’s Center of Innovation for Veteran-Centered and Value-Driven Care developed an Implementation and Adaptation Guide, which is intended to help anyone planning to implement a new practice into any healthcare setting. The Guide provides tools and steps to implement and adapt an evidence-based practice, program, or intervention (these terms are used interchangeably in the Guide). In addition, it will work for multiple levels (e.g., hospital transitions, community care, primary care teams) within different settings. Examples from previous work by investigators with implementing care coordination interventions are used in the Guide, but these steps can be used to adapt and implement any evidence-based practice into a healthcare environment.
The Implementation and Adaptation Guide follows the steps outlined in the Implementation Roadmap, developed by VA/HSR&D’s Quality Enhancement Research Initiative (QUERI). The Roadmap facilitates uptake of an intervention or evidence-based practice through three phases: 1) Pre-implementation/Planning, 2) Implementation/Program Delivery, and 3) Sustainment/Scale-up.
