Project Modification Requests

Health Services Research and Development (HSR&D) communicates Project Modification Request Notifications via the original modification pdf request form, indicating approval or disapproval, signed by HSR&D leadership and with comments from Central Office will be sent to the field along with budget/pink sheets as applicable.

“To be funded studies” that are still in the Just-In-Time (JIT) review that require changes must use the prefunding modification JIT process. If a prefunding modification is needed, please contact your Scientific Program Manager for the Prefunding Modification form to be added to JIT.

Black out dates: August and September. All current fiscal year requests with a budget or extension, must be submitted by August 1 due to closing of financial databases.
