Role of Kidney Disease in an Elderly High-risk Population

BACKGROUND/RATIONALE:
CKD is a highly prevalent, complex, and debilitating chronic illness. Elderly Veterans The elderly are at especially high risk for CKD and its complications. However, little is known about the quality of CKD care within VA or the effect of CKD on utilization and outcomes.

OBJECTIVE(S):
1. To determine and to compare healthcare utilization and health outcomes among elderly Veterans with CKD and without CKD. 2. To assess adherence of three important CKD consensus practice guideline (CPG) treatments (optimal blood pressure control, appropriate glycemic control, use of ACE-I/ARB) among elderly Veterans with CKD. 3. To examine the association and variability between healthcare utilization, health outcomes, and important CKD CPG treatments among elderly Veterans with CKD by sociodemographic and clinical factors.

METHODS:
Retrospective cohort analysis of elderly individuals with and without CKD who received healthcare at VA medical facilities throughout the United States from FY02 through FY05. Three CKD treatments will be measured: 1) blood pressure control, 2) glycemic control, 3) use of ACE-I/ARB. Multiple measures of healthcare use will be assessed including hospitalizations. The three health outcomes examined will consist of mortality, adverse cardiovascular events, and incident end-stage kidney disease (ESKD). In addition to the role of CKD, the role of predisposing (e.g., demographics), enabling (e.g., socioeconomic), and need (e.g., health status) factors toward the outcomes will be evaluated. To examine the association between healthcare utilization and guideline treatments as well as the influence of other key factors, multivariable regression analysis will be used.

IMPACT:
In aggregate, these results stress that a large number of VA healthcare users are older patients with CKD, that their CKD care is often inconsistent with CPGs, and that current CPG hypertension treatment goals may not optimize health outcomes for elderly Veterans with CKD. The remaining objective of this study that is being completed will improve the understanding of the resource consequences of CKD and its treatments.
