Abstract:

The Health Disparities Cancer Collaborative (HDCC) was a demonstration program to increase cancer screening and follow-up among underserved populations seen at community health centers from 2003 to 2005. HDCC interventions used components of the Chronic Care Model (CCM), including (1) self-management support, (2) decision support, (3) delivery system design, (4) clinical information systems, and (5) community outreach to reorganize care delivery. Our aim was to evaluate the influence of HDCC participation upon CCM implementation in community health centers.
