Description: We will present the evidence from studies comparing the effectiveness and harms of pharmacist-led chronic disease management interventions to usual care in community-dwelling adults with chronic diseases. We will report findings for clinical outcomes, resource use outcomes, and medication outcomes and identify future research needs. The intended audience includes: pharmacists, pharmacy researchers, physicians, PACT team members.
