Abstract:

The majority of rural veterans access both VA and non-VA healthcare, yet substantial barriers to information exchange and care coordination across systems remain. We will discuss patterns of dual use, perceptions of co-management from perspectives of community-based primary care providers, VA primary care providers and veterans, and evolving toolkits designed to build a 'Patient Centered Medical Duplex' for rural veterans.
