Abstract:

Description: The 5-site PACT Intensive Management (PIM) evaluation has found that PIM teams were able to engage complex high-risk patients in their health care and increase patient trust in VA providers. Because patients at highest risk for hospitalization or mortality are generally managed in PACT rather than specialty PACTs, some of the PIM case management activities may be useful for PACT. Our objective was to identify which PIM components could be incorporated into PACT team workflow, and what additional resources, trainings, and staff PACTs would need to better manage their high-risk patients. Intended audience: VHA policymakers, administrators, clinicians, and researchers, and other disciplines caring for complex patients in primary care.
