Brief Description: Research from the Delirium CREATE resulted in multiple significant findings for clinicians and nurses that improve the quality of patient care. Study findings identified content for a Dashboard for the EHR and note templates to be used by residents and clinicians when admitting patients. Researchers created health factors related to delirium whichwill auto populate when the nation-wide system goes live. Risk Score generation has been validated and completed by VISN 1. Note templates for nurses alert them when there are high risk levels and medication concerns. These tools will increase nurse comfort in communicating with clinicians about Veterans condition. Adoption by VA Facilities: Pending nationwide implementation.
