Abstract:

The objective of this report is to present findings from Aim 1 of the QUERI Partnered-Evaluation of GeriPACT Implementation. In this report, we present information pertaining to GeriPACT structures and robust team-based care dimensions (i.e., team functioning and Patient-Aligned Care Team Components). These data will be used in the next phase of the evaluation to assess whether structures and team-based care dimensions are related to patient-level outcomes. We expect that the findings presented in the report will be helpful in updating the operations manual and assisting in making policy decisions.
