Description: The presenters will describe reasons for using data-driven methods to assign patients at high-risk for hospitalization to groups with similar clinical profiles. They will share approaches to building group models that are statistically robust, clinically informative, and equitable. This presentation will illustrate how healthcare system data for each group could inform group-tailored intervention design. The presenters will then describe new VA applications developed to inform the care of high-risk patient groups. Target audience: Researchers, Analysts, Clinical and Operations Leaders, Clinical Staff caring for complex patient populations
