Description: Patient Care Assessment System (PCAS) 3.0 was released last year, and included new patient appointment information, the Office of Patient-Centered Care and Cultural Transformation’s “Personal Health Inventory”, additional clinical information, risk filters, and a dynamic note creator to facilitate CPRS documentation. In 2019, PCAS will implement two major new features: (1) case management filters, and (2) access to PCAS for any VA provider, regardless of PCMM participation. With this expansion, PCAS continues to evolve as a tool to support the transitions of care our patients experience. Intended audience: all VA care providers, especially primary care and case managers, as well as anyone interested in high risk patient identification and care coordination strategies.
