Description: Hospitals and health systems are increasingly held accountable for outcomes after hospital discharge, yet high-value practices are difficult to find. Using the translational medicine paradigm (T1 to T4), Dr. Burke will discuss active issues regarding post-discharge and transitional care reflecting on his own CDA work.
